House Speaker Mike Johnson announced on Tuesday that major cuts to the Affordable Care Act’s Medicaid expansion are no longer on the table. This means that the remaining options for cuts would disproportionately affect states that supported Trump in the 2024 election. While Republicans are still considering smaller changes to Medicaid, they have only two major policy options that could result in the $880 billion in spending cuts that the House committee overseeing Medicaid has been tasked with finding. One option is to significantly reduce funding for the Obamacare Medicaid expansion, which could save $710 billion over the next decade. However, this would mostly impact wealthy, Democratic-led states. The other option is to limit the use of a tax loophole that allows states to increase federal spending on Medicaid, which could save $668 billion. This would mostly affect poorer, Southern states. Whichever option is chosen, the states that are hit hardest would face significant budget shortfalls. To make up for this, some states may have to drop Medicaid coverage for lower-income adults, cut hospital payments, or reduce funding for other government priorities.
